Fell Behind on Your Pelvic Rehab Exercises? Why You Shouldn't Cancel Your Next Physio Appointment

It is one of the most common scenarios I see in the clinic, and yet, it is the one patients feel the most unnecessary shame about.

You finally take the leap and come in to get your pelvic floor checked. We sit down, map out a clear pathway to tackle your Chronic Pelvic Pain Syndrome (CPPS), Hard Flaccid, or erectile dysfunction. We do some hands-on work, and for the first time in months—maybe years—you leave feeling some genuine relief. You go home with a tailored exercise programme, fully intending to do it.

But then, life happens. Work gets busy, the kids get sick, or the sheer mental fatigue of dealing with chronic symptoms drains your motivation. A week passes, and you haven't done a single stretch, breathing drill, or nerve glide.

The guilt sets in. You think, "I've failed. Karl is going to be disappointed. I should just cancel my next session until I've actually done the work."

Let me stop you right there. This is a zero-guilt clinic. We are not here to name and shame you, give you a rap on the knuckles, or make you feel worse than you already do. When you skip your exercises, the worst thing you can do is avoid your follow-up. Here is why.

The Clinical 'Why': Your Tissues Are Already Changing

To understand why you shouldn't cancel, you need to understand the neuromuscular mechanics of what we are actually treating.

When you suffer from conditions like CPPS or Hard Flaccid, your pelvic floor is trapped in a chronic state of protective muscle guarding. Imagine someone standing with a heavy foot on a garden hose. The pressure restricts the flow of water. In your pelvis, that "foot" is a tight, hypertonic muscle spasm compressing delicate nerves (like the pudendal nerve) and restricting vital blood flow to your penis and surrounding tissues. This is exactly why endless rounds of antibiotics from your GP have failed—you cannot cure a mechanical, nerve-compression issue with a pill.

During your first session, we use manual, hands-on therapy to essentially lift that foot off the garden hose. This sends new, safe signals to your nervous system, giving those compressed nerves a chance to breathe.

Even if you didn't do your homework, that physical process has already started. The ball is rolling. Your tissues are currently adapting to those manual changes we made in the clinic. The exercises are simply the tool we use to keep the foot off the hose permanently. If you haven't done them, we haven't lost the war—we just need to keep up the momentum with another hands-on session.

The Delaney Approach: Adapting to Your Life

If you show up to your appointment having done zero rehab, we simply pivot. Here are the three principles I want you to remember when things don’t go to plan:

  • Number One: Don't Panic. It is completely understandable. We are asking your body and brain to make healthy, fundamental changes to habits you’ve likely held for years. Rewiring a nervous system that has been locked in "fight or flight" takes time, patience, and a bit of grace. A missed week is just a blip on the radar.

  • Number Two: Don't Postpone. Cancelling your appointment halts your progress. As we established, the manual therapy we did in your previous session has already primed your tissues for change. Delaying your next visit allows that "foot" to creep back down onto the garden hose. Show up, let us continue the hands-on therapy, and keep your physical recovery moving forward.

  • Number Three: Let's Talk About It. If you didn't do the programme, it usually means the programme wasn't right for your current lifestyle. So, let's discuss it. What were the obstacles? Was the routine too long? Were you unsure of the technique? My job as your physiotherapist is to adapt the plan. The goal is to modify your rehab so it seamlessly fits into your day-to-day life, making it easier to execute so you can finally feel good about your progress.
