Recently, I had the absolute privilege of joining the Solihull Prostate Cancer Support Group for an evening discussion. Rather than standing at the front of the room with a clinical PowerPoint presentation, we pulled up a few chairs, grabbed a microphone, and had an open, "fireside" chat about the physical realities of prostate cancer treatment.

What struck me most wasn’t just the resilience of the men in the room, but how many shared the exact same frustrations. Time and time again, I hear men say, “The cancer is gone, but I feel like my quality of life hasn't come back.”

Whether you are preparing for a prostatectomy or are months into your recovery, there is a widespread misconception that changes to your continence and sexual function are just a permanent "new normal" you have to accept.

At Delaney Physiotherapy in Coventry, we know that isn't true. Here is a breakdown of what we discussed in Solihull, and how a specialized, mechanical approach to pelvic health can help you regain control.

Beyond "Just Do Your Kegels": The Active Mechanic Approach

One of the biggest myths in prostate cancer recovery is that if you leak, you just need to do hundreds of Kegel exercises to "strengthen" your pelvic floor.

The reality is that after a major trauma like a prostatectomy, the pelvic floor muscles often go into a protective spasm—much like a clenched fist. You cannot strengthen your way out of a cramp. Instead of just handing out a generic sheet of exercises, a Specialist Men's Pelvic Health Physiotherapist acts like an active mechanic. We look at your nervous system, your muscle tension, and your mechanics to help your body let go of that protective tension before we rebuild your strength.

Pre-hab: Setting the Stage Before Surgery

We spent a good portion of the evening discussing the importance of "pre-hab." If you know you are scheduled for a radical prostatectomy, seeing a specialist beforehand is one of the best investments you can make in your recovery.

  • Mapping the muscles: We ensure you know exactly how to engage and—crucially—relax your pelvic floor before the surgical trauma occurs.

  • Establishing baseline function: Understanding your mechanics beforehand makes it significantly easier to "wake up" the correct pathways after your catheter is removed.

Tackling Incontinence: Retraining the System

Leaking urine (incontinence) is the most common fear men have post-surgery. It can be incredibly isolating, but it is fundamentally a mechanical roadblock. Using advanced neuromuscular retraining, we work to properly coordinate the sphincters and the surrounding pelvic musculature. The goal is to move you away from relying on pads and back to trusting your own body's mechanics, whether you are lifting weights, sneezing, or just getting up from a chair.

Restoring Sexual Function: An Open Conversation

Erectile dysfunction and changes to sexual health are often the "elephant in the room" that men are hesitant to discuss. During our fireside chat, we tackled this head-on. Just like continence, sexual function relies on adequate blood flow, nerve health, and properly functioning pelvic floor muscles. Targeted physiotherapy provides non-drug, active mechanical strategies to maximize your recovery, improve rigidity, and help you regain your confidence.
